The wound dictates the terms. How much we are going to remove or what kind of surgery the patient is going to undergo will be decided by the extent of damage happened in the foot.
Debridement
This procedure involves removing the infected skin and tissue from the foot ulcer after assessing the extent of soft tissue damage. During this procedure, a surgical blade is used to remove unhealthy tissues - after which is wound is washed with a saline solution. It is performed as a day care procedure in most of the cases. The patient can go home on the same day .
Bone curettage
Here, in this surgery the surgeon will remove the infected part of the bone along with the unhealthy soft tissues.
Amputations
Removing a part of the foot either complete or incomplete removal of toes.
Even the half of the foot may be removed in severely infected cases (forefoot amputations)
In severe sepsis, one may undergo Major amputations (Below knee or above knee level )as a life saving procedure.
Vascular surgery (angioplasty and bye pass)
People with diabetes have lower extremity arterial disease that reduce the flow of blood in legs and feet. This prevents the foot wound or injury from getting the nutrients it needs. Doctors attached to Madutal Foot Care Center can perform minimally invasive surgery to improve blood flow.
Peripheral angioplasty
The surgeon introduces a small tube into the artery by doing a small incision usually in the groin( upper thigh)region and clears a clogged artery by cutting the waxy substance composed of fat (atheromatous plaques) in the blood. There are more than ten different ways of removing the plaques which is obstructing the artery.
The patient may return home the same day of surgery or may need to stay in hospitals for 1 or 2 days - depending on the procedure performed.
In very difficult cases, the surgeon may try both Endo vascular and open bye pass surgery to get the blood flow to the foot.
Many people with diabetes will develop wounds in the foot and are unaware of the problem until it reaches an advanced stage. They avoid seeking treatment due to the fear of surgery. Through regular checkups, early diagnosis and self-care, one can prevent foot ulcer surgery.
